Applications for Admissions 2012-2013

Please print the grade-appropriate Application Packet.  

Submit the completed application, fee, copy of the birth certificate and baptism certificate (if applicable) directly to IHM School.

The top portion of the Release of Records and Student Evaluation Forms should be completed by the parent.  They should then be·given to your child’s current school who will forward them directly to IHM School.

Catholic applicants should give the Parish Verification Form to your parish office. They will complete and forward the form directly to IHM School.

Parish Verification Form

 All materials should be received by IHM by February 1st.
